Replacement Diploma​ Policy


Replacement Diploma Policy (as of 09/29/2023):

Thank you for your inquiry regarding a replacement diploma. Please be aware of the following conditions:

  • Replacement diplomas are issued for a fee of $25.00, with a check or money order made payable to Providence College.
  • Requests can only be initiated by alumnae. Third parties cannot request diplomas.
  • The original diploma format, if different from the format currently used by the college, cannot be duplicated. The replacement diploma you receive will be similar to that awarded to the current year’s graduating class.

Instructions for requesting a replacement diploma:

  • Please print and then fill out the Replacement Diploma Request Form.
    • If you are replacing your diploma due to a name change, please complete the Name Change Request Form. Email the Name Change Form and a copy of your photo ID to records@providence.edu before submitting your replacement diploma request. Include the words “[PC-Encrypt]” in the email’s subject line so that your message is sent securely. You must also return the original diploma before a replacement is issued.
  • Diplomas will be sent to the address provided on the form. Please be sure that it is the correct address.
  • The form must be notarized. Further, replacement diploma forms will only be processed if they have an original handwritten signature (cannot be typed).
  • Include a check or money order for $25.00 with the form, made payable to Providence College.
  • Drop the form and check off at our office, or mail both to:

  • Emailed submissions of the Replacement Diploma Request Form will not be processed.
  • Please be aware that ordering a replacement diploma takes 6 to 8 weeks from the office receiving the check.
  • Diplomas will be mailed in an oversized envelope with stiff cardboard inside. They will not fit into a standard mailbox. Please alert your mail carrier if you are electing the mail option.
