This page contains links to several of the more common forms utilized by students and faculty advisors. Each of these forms is available in printable format. All change requests are subject to review. Questions about these forms should be directed towards the office listed on the individual form. To complete any of these forms, please download the form and save it to your computer first, then fill it out. If you fill out the form first and then download the form, it will download blank.
- Change of Major/Minor/Concentration Request Form
- You must meet with the department designee of the major(s)/minor(s)/concentration(s) you wish to add/retain/delete PRIOR to submitting this form.
- Please Note: For specific information on the process to change a business major/minor see the PCSB website here. All other changes may be requested by following the instructions on the form.
- Instructions for Students – Incomplete forms will be returned to you and will result in a delay in processing.
- Check the box acknowledging that you have met with all necessary departments before submitting the form.
- Step 1: Indicate your intent to add new major(s)/minor(s)/concentration(s).
- Step 2: You must indicate your intent to retain or delete each of your current major(s)/minor(s)/concentration(s), listed above.
*THIS STEP IS REQUIRED. FAILURE TO COMPLETE THIS STEP WILL RESULT IN A DELAY IN PROCESSING.* - Step 3: Click on the yellow Student Signature box to sign and date the form.
- Step 4: Click “Submit Form”.

- Over-Enrollment/Late Add Form (Fall 2022)
- Please return the completed form with all signature approvals the Instructor and Department Chair/Program Director giving permissions to Harkins Hall 310.
- Please, only use this form for over-enrollments until September 13, 2022 for late additions, all other changes through September 6, 2022 can be made in CyberFriar. Class drops can be made in CyberFriar until September 13.
- September 6, 2022, 11:59 PM – Last day to add a course in CyberFriar.
- For juniors and seniors only, the last day for CyberFriar registration into an SCE class is August 28, 2022.
- September 13, 2022, 11:59 PM – Last day to drop a course in CyberFriar.
- September 14, 2022- There is a late charge of $150.00 to add a course. Any student looking to add a course at this point needs to use a course registration adjustment form. Any dropped courses will be processed with a grade of a ‘WD’ (withdrawal).
- September 6, 2022, 11:59 PM – Last day to add a course in CyberFriar.
- Change of Name and/or Change of Address Form
